and will be happy to check coverage for you and if you decide to start treatment in our office, we will also be more
than happy to submit all insurance claims for you. Our office will do everything to help you afford the treatment
you need and want. The forms of payment accepted in our office are checks, cash, and credit cards. Cancellation
If you are unable to keep your appointment, we ask
that you provide us with at least 24 hours notice and will be happy to reschedule your appointment for another day. We ask
for this advance notice so that we can offer this appointment to another patient.
